Ajovy vs Aimovig

Ajovy (fremanezumab) and Aimovig (erenumab-aooe) are both anti-CGRP-pathway antibodies reviewed for the prophylaxis of migraine in adults who experience at least 4 migraine days per month. Regulators reviewed Ajovy based on studies showing a reduction from baseline for primary endpoints with fremanezumab monthly dosing (episodic migraine) of 3-5, and for chronic migraine of 3-5, as well as with fremanezumab quarterly dosing of 3-5. Aimovig was reviewed based on studies demonstrating a difference in LSM for MMD vs placebo of -2.46.

It is crucial to note that these drugs were studied in separate trials with different designs, populations, and timepoints. For Ajovy, an adjusted mean treatment difference in monthly average number of migraine days vs placebo (fremanezumab 225/225/225 mg) was -1.8. For Aimovig, an adjusted odds ratio for MMD responder rate vs placebo was 2.34. The trials differed in their specific endpoints, patient populations, timeframes, and comparator arms. Therefore, the figures presented are not head-to-head comparisons, and neither drug is presented as superior to the other.
